Long-term care utilization within older adults with schizophrenia: Associated factors in a multicenter study

Introduction
Data are scarce regarding the clinical factors associated with utilization of long-term care facilities among older adults with schizophrenia.
ObjectivesIn this multicenter study, we sought to examine potential clinical differences between older adults with schizophrenia who are living in a long-term care facility and their community-dwelling counterparts.
MethodsWe used data from the French Cohort of individuals with Schizophrenia Aged 55-years or more (CSA) study, a large multicenter sample of older adults with schizophrenia (N = 353).

ResultsResults from the multivariable binary logistic regression analysis including all variables that had a significant association in univariate analyses (i.e., p < 0.05) revealed that older age (Adjusted odds ratio (AOR) [95%CI]=1.08 [1.03–1.13]), depression (AOR [95%CI]=1.97 [1.06–3.64]), lower MMSE (AOR [95%CI]=0.94 [0.88–0.99]) and GAF scores (AOR [95%CI]=0.97 [0.95–0.99]), living in an area comprising more than 1000 inhabitants per km2 (AOR [95%CI]=2.81 [1.37–5.80]), having consulted a general practitioner in the past year (AOR [95%CI]=0.28 [0.0.14–0.56]), and a greater lifetime number of hospitalizations in a psychiatric department (AOR [95%CI]=2.30 [1.18–4.50]) were significantly and independently associated with long-term care utilization among older adults with schizophrenia . In the multivariable logistic regression model, the variance inflation factor (VIF) and tolerance values of each predictor variable were respectively lower than 2.5 and higher than 0.2, supporting that multicollinearity was not a concern in our analysis.
ConclusionsIn a multicenter sample of 353 older adults with schizophrenia, we found that ong-term care utilization was significantly and independently associated with depression, lower cognitive and global functioning, greater lifetime number of hospitalizations in a psychiatric department, not having consulted a general practitioner in the past year, urbanicity and older age. Patients living in a long-term care facility appear to belong to a distinct group, marked by a more severe course of illness with higher level of depression and more severe cognitive deficits.
Despite its limitations, this study contributes to gain more specific knowledge about this specific understudied population. Our study highlights the need of early assessment and management of depression and cognitive deficits in this population and the importance of monitoring closely this vulnerable population.

Haemoproteus spp. are dipteran-borne protozoa that infect erythrocytes and reticulo-endothelial cells of birds. These parasites are not usually transmitted between birds belonging to different orders. The suborder Lari (order Charadriiformes) comprises ~170 avian species, the majority of which are aquatic, including gulls, terns, auklets, murres and skuas, among others. In spite of the diversity of this avian group, there is limited known diversity of haemosporidian parasites, with only 4 recorded Haemoproteus morphospecies thus far. We examined the blood smears of 21 kelp gulls (Larus dominicanus) captured at a breeding colony in South Africa, as well as Haemoproteus-positive archival blood smears of 15 kelp gulls and 1 Hartlaub's gull (Larus hartlaubii) sampled while under care at seabird rehabilitation facilities in South Africa. Haemoproteus sp. infection was detected in 19% of wild-caught kelp gulls. All parasites from the gulls were morphologically identified as Haemoproteus jenniae, a species previously recorded in Lari birds at the Galapagos Islands (Ecuador), Rocas Atoll (Brazil) and Poland. Gene sequencing uncovered a new cytochrome b lineage, LARDOM01, which was closely related to the previously reported H. jenniae lineage CREFUR01. Additionally, we evaluated a hapantotype blood smear of Haemoproteus skuae, which had been described infecting a brown skua (Catharacta antarctica) in South Africa. We provide a redescription of H. skuae and discuss the morphological characters distinguishing it from H. jenniae. Further research is necessary to improve our knowledge about the host and geographic distribution, health effects and phylogeny of H. jenniae and H. skuae.

This article is a clinical guide which discusses the “state-of-the-art” usage of the classic monoamine oxidase inhibitor (MAOI) antidepressants (phenelzine, tranylcypromine, and isocarboxazid) in modern psychiatric practice. The guide is for all clinicians, including those who may not be experienced MAOI prescribers. It discusses indications, drug-drug interactions, side-effect management, and the safety of various augmentation strategies. There is a clear and broad consensus (more than 70 international expert endorsers), based on 6 decades of experience, for the recommendations herein exposited. They are based on empirical evidence and expert opinion—this guide is presented as a new specialist-consensus standard. The guide provides practical clinical advice, and is the basis for the rational use of these drugs, particularly because it improves and updates knowledge, and corrects the various misconceptions that have hitherto been prominent in the literature, partly due to insufficient knowledge of pharmacology. The guide suggests that MAOIs should always be considered in cases of treatment-resistant depression (including those melancholic in nature), and prior to electroconvulsive therapy—while taking into account of patient preference. In selected cases, they may be considered earlier in the treatment algorithm than has previously been customary, and should not be regarded as drugs of last resort; they may prove decisively effective when many other treatments have failed. The guide clarifies key points on the concomitant use of incorrectly proscribed drugs such as methylphenidate and some tricyclic antidepressants. It also illustrates the straightforward “bridging” methods that may be used to transition simply and safely from other antidepressants to MAOIs.

We summarize some of the past year's most important findings within climate change-related research. New research has improved our understanding of Earth's sensitivity to carbon dioxide, finds that permafrost thaw could release more carbon emissions than expected and that the uptake of carbon in tropical ecosystems is weakening. Adverse impacts on human society include increasing water shortages and impacts on mental health. Options for solutions emerge from rethinking economic models, rights-based litigation, strengthened governance systems and a new social contract. The disruption caused by COVID-19 could be seized as an opportunity for positive change, directing economic stimulus towards sustainable investments.
Technical summaryA synthesis is made of ten fields within climate science where there have been significant advances since mid-2019, through an expert elicitation process with broad disciplinary scope. Findings include: (1) a better understanding of equilibrium climate sensitivity; (2) abrupt thaw as an accelerator of carbon release from permafrost; (3) changes to global and regional land carbon sinks; (4) impacts of climate change on water crises, including equity perspectives; (5) adverse effects on mental health from climate change; (6) immediate effects on climate of the COVID-19 pandemic and requirements for recovery packages to deliver on the Paris Agreement; (7) suggested long-term changes to governance and a social contract to address climate change, learning from the current pandemic, (8) updated positive cost–benefit ratio and new perspectives on the potential for green growth in the short- and long-term perspective; (9) urban electrification as a strategy to move towards low-carbon energy systems and (10) rights-based litigation as an increasingly important method to address climate change, with recent clarifications on the legal standing and representation of future generations.

Isotopic composition of leaf carbon (δ13C) and nitrogen (δ15N) is determined by biotic and abiotic factors. In order to determine the influence of leaf habit and site on leaf δ13C and δ15N in the understorey of two Atlantic forests in Brazil that differ in annual precipitation (1200 and 1900 mm), we measured these isotopes in the shaded understorey of 38 tropical tree species (20 in the 1200-mm site and 18 in the 1900-mm site). Mean site values for δ15N were significantly lower at the 1200-mm site (−1.4‰) compared with the 1900-mm site (+3.0‰), and δ13C was significantly greater in the 1200-mm site (−30.4‰) than in the 1900-mm site (−31.6‰). Leaf C concentration was greater and leaf N concentration was lower at 1200-mm than at 1900-mm. Leaf δ15N was negatively correlated with δ13C across the two sites. Leaf δ13C and δ15N of evergreen and deciduous species were not significantly different within a site. No significant phylogenetic signal for any traits among the study species was found. Overall, site differences were the main factor distinguishing traits among species, suggesting strong functional convergence to local climate and soils within each site for individuals in the shaded understorey.

A selection of elements (Bi, Ca, Cd, Co, Cu, Mn, Na, Sr, U, V, Zn) were measured by high-resolution inductively coupled plasma sector-field mass spectrometry in firn- and ice-core samples from Law Dome, Antarctica, corresponding to the period 4500 BC to AD 1989. Concentrations of rock dust and sea salts were calculated for each sample and then used to determine concentrations of each element originating from crustal and marine aerosol emissions, respectively. Where calculated contributions from crustal and marine aerosol sources failed to account for the total measured concentration of an element, the remainder was apportioned to volcanic and/or anthropogenic sources and defined as an enrichment. On this basis, it was determined that Bi and Cd concentrations in Law Dome ice are overwhelmingly influenced by volcanic emissions (enrichments 150–250x crustal and marine inputs); Co, Cu, Pb and Zn concentrations in Law Dome ice are largely influenced by volcanic emissions (enrichments 16–36x crustal and marine inputs); and Mn, Sr, U and V concentrations in Law Dome ice are minimally influenced by volcanic emissions (enrichments 1.5–4x crustal and marine inputs). During the 20th century, enrichments of Pb and Cu concentrations were observed to be greater than in earlier centuries, consistent with increasing anthropogenic emissions of Pb and Cu in the Southern Hemisphere over that period.

Mineral requirements of pregnant dairy goats are still not well defined; therefore, we investigated the net Ca, P, Mg, Na and K requirements for pregnancy and for maintenance during pregnancy in two separate experiments. Experiment 1 was performed to estimate the net Ca, P, Mg, Na and K requirements in goats carrying single or twin fetuses from 50 to 140 days of pregnancy (DOP). The net mineral requirements for pregnancy were determined by measuring mineral deposition in gravid uterus and mammary gland after comparative slaughter. In total, 57 dairy goats of two breeds (Oberhasli or Saanen), in their third or fourth parturition, were randomly assigned to groups based on litter size (single or twin) and day of slaughter (50, 80, 110 and 140 DOP) in a fully factorial design. Net mineral accretion for pregnancy did not differ by goat breed. The total daily Ca, P, Mg, Na and K requirements for pregnancy were greatest in goats carrying twins (P<0.05), and the requirements increased as pregnancy progressed. Experiment 2 was performed to estimate net Ca, P, Mg, Na and K requirements for dairy goat maintenance during pregnancy. In total, 58 dairy goats (Oberhasli and Saanen) carrying twin fetuses were assigned to groups based on slaughter day (80, 110 and 140 DOP) and feed restriction (ad libitum, 20% and 40% feed restriction) in a randomized block design. The net Ca, P and Mg requirements for maintenance did not vary by breed or over the course of pregnancy. The daily net requirements of Ca, P and Mg for maintenance were 60.4, 31.1 and 2.42 mg/kg live BW (LBW), respectively. The daily net Na requirement for maintenance was greater in Saanen goats (11.8 mg/kg LBW) than in Oberhasli goats (8.96 mg/kg LBW; P<0.05). Daily net K requirements increased as pregnancy progressed from 8.73 to 15.4 mg/kg LBW (P<0.01). The findings of this study will guide design of diets with adequate mineral content for pregnant goats throughout their pregnancy.

Why do authoritarian regimes try to improve the quality of their governance? In the absence of democratic institutions to monitor, reward and punish their performance, authoritarian politicians are normally expected to seek their self-interest through corruption and rewards to cronies, rather than providing for the public welfare. However, the Chinese state has actively promoted improved governance in recent years, with greater attention to quality of life issues to balance the primary focus on sustaining rapid economic growth. This paper analyses intra-national variation in the provision of public goods in urban China and the impact of public goods on regime support. Does better governance lead to higher levels of public support for the regime, even in the absence of democratic elections? Our evidence suggests that it does, with a greater impact for the local level than for the centre.

We present a catalogue containing the redshifts of 3 660 X-ray selected targets in the XXL southern field. The redshifts were obtained with the AAOmega spectrograph and 2dF fibre positioner on the Anglo-Australian Telescope. The catalogue contains 1 515 broad line AGN, 528 stars, and redshifts for 41 out of the 49 brightest X-ray selected clusters in the XXL southern field.

Lenses in laparoscopes, arthroscopes, and laryngoscopes fog during closed body surgery due to humidity from bodily fluids and differences between body and operating room temperatures.1,2 Surgeons must repeatedly remove, clean, and reinsert scopes that are obscured by fog. As a result, surgery duration, infection risks, and scarring from air exposure increase.3,4 Current methods to address fogging introduce other complications. Acidic alcohol-based coatings scar tissue and quickly evaporate, and heated lenses require reheating every 5 to 20 minutes.3,4 This paper presents a new super-hydrophilic, biocompatible, non-toxic, pH neutral (7.2-7.4), and long-lasting anti-fog coating called VitreOx™.5-7 VitreOx™ can be used wet or dry, without use of alcohol, heat, or fluid evacuation. When applied as a liquid, it easily espouses lenses’ surfaces and edges, and dries within seconds to form a permanently super-hydrophilic surface on silica and polymer surfaces. VitreOx™ avoids current shortfalls by eliminating frequent reapplications, avoiding reapplication for surgeries lasting up to 72 hours.
VitreOx™'s anti-fog properties can be explained by nucleation and growth theory for thin films condensation: 1) 3-D droplets, resulting in fogging; 2) 2-D sheets resulting in a flat transparent film; or 3) mixed 3-D on 2-D, resulting in optical distortion. On hydrophobic surfaces (e.g. lenses), condensation occurs with fogging via spherical 3-D droplets, as in the Volmer-Weber model. 3-D droplets scatter light in all directions through refraction yielding opaque or translucent films (fog). VitreOx™ applied to hydrophobic lenses renders them super-hydrophilic. Similar to the 2-D Frank Van-der-Merwe Growth Mode, condensation with uniform wetting yields transparent 2-D films that do not distort optical images transmission.
In vitro and in vivo studies of VitreOx™ were conducted to measure performance and duration of anti-fog effectiveness and bio-compatibility. In vitro tests spanned from 3 to 72 hours over a 3-year range. Side-by-side in vivo gastro-endoscopies were conducted on Yucatan™ swine for 90 minutes using 1) VitreOx™, 2) bare lens, and 3) Covidien Clearify™ surfactant with warmer. VitreOx™ coated lenses did not fog nor need reapplication for 90 minutes, while Covidien Clearify™ lasted 38 minutes without fogging, requiring retreatment. No adverse reaction was observed on swines exposed toVitreOx™, in surgery and 12 months thereafter.

Placental lipoprotein lipase (LPL) is crucial for placental lipid transfer. Impaired LPL gene expression and activity were reported in pregnancies complicated by gestational diabetes mellitus (GDM) and intra-uterine growth restriction. We hypothesized that placental LPL DNA methylation is altered by maternal metabolic status and could contribute to fetal programming. The objective of this study was thus to assess whether placental LPL DNA methylation is associated with GDM and both maternal and newborn lipid profiles. Placenta biopsies were sampled at delivery from 126 women including 27 women with GDM diagnosed following a post 75 g-oral glucose tolerance test (OGTT) between weeks 24 and 28 of gestation. Placental LPL DNA methylation and expression levels were determined using bisulfite pyrosequencing and quantitative real-time PCR, respectively. DNA methylation levels within LPL proximal promoter region (CpG1) and intron 1 CpG island (CpGs 2 and 3) were lower in placenta of women with GDM. DNA methylation levels at LPL-CpG1 and CpG3 were also negatively correlated with maternal glucose (2-h post OGTT; r=–0.22; P=0.02) and HDL-cholesterol levels (third trimester of pregnancy; r=–0.20; p=0.03), respectively. Moreover, we report correlation between LPL-CpG2 DNA methylation and cord blood lipid profile. DNA methylation levels within intron 1 CpG island explained up to 26% (r⩽–0.51; P<0.001) of placental LPL mRNA expression variance. Overall, we showed that maternal metabolic profile is associated with placental LPL DNA methylation dysregulation. Our results suggest that site-specific LPL epipolymorphisms in the placenta are possibly functional and could potentially be involved in determining the future metabolic health of the newborn.

Transition metal nitrides containing metal ions in high oxidation states are a significant goal for the discovery of new families of semiconducting materials. Most metal nitride compounds prepared at high temperature and high pressure from the elements have metallic bonding. However amorphous or nanocrystalline compounds can be prepared via metal-organic chemistry routes giving rise to precursors with a high nitrogen:metal ratio. Using X-ray diffraction in parallel with high pressure laser heating in the diamond anvil cell this work highlights the possibility of retaining the composition and structure of a metastable nanocrystalline precursor under high pressure-temperature conditions. Specifically, a nanocrystalline Hf3N4 with a tetragonal defect-fluorite structure can be crystallized under high-P,T conditions. Increasing the pressure and temperature of crystallization leads to the formation of a fully recoverable orthorhombic (defect cottunite-structured) polymorph. This approach identifies a novel class of pathways to the synthesis of new crystalline nitrogen-rich transition metal nitrides.
Risk factors for cholera transmission in Haiti during inter-peak periods: insights to improve current control strategies from two case-control studies

Two community-based density case-control studies were performed to assess risk factors for cholera transmission during inter-peak periods of the ongoing epidemic in two Haitian urban settings, Gonaives and Carrefour. The strongest associations were: close contact with cholera patients (sharing latrines, visiting cholera patients, helping someone with diarrhoea), eating food from street vendors and washing dishes with untreated water. Protective factors were: drinking chlorinated water, receiving prevention messages via television, church or training sessions, and high household socioeconomic level. These findings suggest that, in addition to contaminated water, factors related to direct and indirect inter-human contact play an important role in cholera transmission during inter-peak periods. In order to reduce cholera transmission in Haiti intensive preventive measures such as hygiene promotion and awareness campaigns should be implemented during inter-peak lulls, when prevention activities are typically scaled back.
